Telfast 60mg contains fexofenadine hydrochloride — a third-generation, non-sedating antihistamine that provides reliable, long-acting relief from hay fever (seasonal allergic rhinitis) and year-round allergies without the drowsiness caused by first and second-generation antihistamines. Fexofenadine does not cross the blood-brain barrier, meaning it has no central nervous system sedation — making it suitable for people who need to drive, operate machinery, or maintain full alertness during the day. First-generation antihistamines (chlorphenamine, promethazine) and some second-generation antihistamines (cetirizine, loratadine in some people) cross the blood-brain barrier and block histamine H1 receptors in the brain — causing sedation, impaired cognition, and reaction times. Fexofenadine is specifically structured to minimise blood-brain barrier penetration, so it blocks peripheral histamine H1 receptors (causing allergy symptom relief) without significant central nervous system effects. Multiple double-blind studies confirm fexofenadine has no more sedation than placebo.
Telfast 60mg is typically used twice daily (morning and evening) for 12-hour dosing intervals. Telfast 120mg is once-daily for adults (used primarily for chronic urticaria/hives). Telfast 180mg is once-daily for seasonal allergic rhinitis. Check with your pharmacist about which strength and dosing schedule is most appropriate for your symptoms and lifestyle.
No — this is an important and often missed interaction. Grapefruit juice, orange juice, and apple juice significantly reduce fexofenadine absorption (by up to 36%) by inhibiting the intestinal transporter (OATP) that brings fexofenadine into the bloodstream. Always take Telfast with water, not fruit juice, for full effectiveness.
Consult your GP or midwife before taking antihistamines during pregnancy or breastfeeding. While fexofenadine is generally considered lower risk than first-generation antihistamines, it is not classified as safe in pregnancy without medical guidance. Loratadine has the most safety data for use during pregnancy.
Aluminium and magnesium hydroxide-containing antacids (like Mylanta, Gaviscon) can reduce fexofenadine absorption by approximately 40%. Take Telfast at least 2 hours before or 2 hours after taking antacids containing aluminium or magnesium.
