NOW D-Mannose provides pure D-mannose — a naturally occurring simple sugar with significant clinical evidence for preventing and reducing recurrent urinary tract infections (UTIs). D-mannose works by a unique and elegant mechanism: it binds to the type 1 fimbriae (adhesion proteins) of E. coli — the bacteria responsible for 80–90% of UTIs — preventing them from adhering to the bladder and urinary tract wall. Unable to attach, the bacteria are flushed out during normal urination. GMP certified, non-GMO, vegan. 120 capsules.
Over 80% of UTIs are caused by E. coli bacteria that have type 1 fimbriae — protein hooks that allow them to attach to mannose receptors on the surface of bladder epithelial cells. D-mannose (a six-carbon sugar) saturates the urine with free mannose molecules that E. coli bind to preferentially. With their fimbriae occupied by free D-mannose, the bacteria can no longer attach to the bladder wall and are flushed out during urination. This mechanism is elegant because it prevents infection without killing bacteria — and therefore does not contribute to antibiotic resistance.
A significant clinical trial (Kranjčec et al., 2014) showed D-mannose was as effective as nitrofurantoin (a common antibiotic) for preventing recurrent UTIs, with a significantly lower rate of side effects. D-mannose is most effective as a prevention strategy for recurrent UTIs (those who get 3+ per year) and as early management when symptoms first appear. For established, moderate-to-severe UTIs, antibiotic treatment remains the standard of care — consult your GP if symptoms are severe or persistent.
D-Mannose is most beneficial for: women with recurrent E. coli UTIs (3+ per year), people wanting to reduce antibiotic use for recurrent UTIs, post-coital UTI prevention, pregnant women (under GP guidance), people with catheter-related UTIs, and as early management at the first sign of UTI symptoms.
For active UTI symptoms: 1.5–2g every 2–3 hours for the first day, then every 3–4 hours for 5 days. For prevention: 500–1000mg twice daily or after sexual activity if intercourse-related UTIs occur. Each capsule provides 500mg — adjust accordingly.
D-mannose is a sugar but is poorly metabolised — only a small fraction is absorbed and used; the majority passes through the kidneys and is excreted in urine (which is where its therapeutic effect occurs). At normal doses, D-mannose has minimal effect on blood glucose levels. However, diabetics should monitor blood sugar when first starting D-mannose supplementation, particularly at higher therapeutic doses.
